Community Health Assessment and Improvement

The Adams County Health Department has completed a Community Health Assessment, an in depth look at the health and well being of Adams County. Through collaboration with other county agencies, service organizations, community meetings, and surveys, the assessment aimed to identify not only the greatest health problems affecting Adams County, but the assets and resources that make it a healthy place to live, work, and raise a family.

​The strengths and weaknesses identified in the health assessment will be used to develop a Community Health Improvement Plan, a roadmap of how we can improve the health and wellbeing of Adams County.
